Position Summary:
The Case Manager at the Juvenile Detention Facility plays a critical role in supporting the rehabilitation and well-being of incarcerated youth. This position involves assessing the needs of juveniles, developing individualized care and rehabilitation plans, and coordinating services that foster their successful reintegration into the community. The ideal candidate will have a strong commitment to youth development, social justice, and restorative practices, with experience in case management, social work, or related fields.
Essential Functions and Responsibilities:
- Conduct comprehensive assessments of detained juveniles, identifying risk factors, needs, strengths, and goals. Develop and implement individualized treatment and care plans in collaboration with youth, families, and interdisciplinary teams.
- Serve as a liaison between the juvenile, their families, legal representatives, specialist, educators, and service providers. Coordinate support services including mental health, educational, legal, and social services.
- Regularly meet with juveniles to review progress, adjust treatment plans, and provide guidance. Document case updates, ensure compliance with facility protocols, and prepare reports for legal proceedings or reviews.
- Advocate for the best interests of the juvenile, ensuring that their rights are upheld, and they have access to necessary services, programs, and resources within the detention center and post-release.
- Develop and implement reintegration plans aimed at a smooth transition back into the community. Collaborate with external agencies to connect youth with housing, employment, education, and therapeutic resources post-release.
- Respond to behavioral, emotional, or psychological crises, de-escalating situations and providing appropriate referrals for immediate care.
- Maintain accurate and confidential records in compliance with local, state, and federal regulations. Ensure all reports and documentation are timely, thorough, and adhere to legal and ethical standards.
- Participates in team meetings with Unit and Medical staff to develop appropriate plans for managing youth.
- Prepares adequate, accurate and timely documentation as required.
- Communicates information to the Court regarding youth assessments as required and requested.
- Retrieves daily the conference request slips from all the Units and facilitate contact with requested party.
- Respond to conference request slip within a 24-hour period during the workweek. Log and track all contacts and attempt on the conference form, place the completed form in the youth's permanent file.
- Conducting individual and/or group sessions with the youth.
- Performs other duties as assigned.
Minimum Requirements:
Bachelors degree in social work, criminal justice, psychology, or a related field (masters degree preferred).
Minimum of Two (2) years of experience in case management, social work, or a related field, preferably with at-risk or incarcerated youth.
Strong knowledge of the juvenile justice system, youth rehabilitation, and restorative justice principles.
Experience working with youth from diverse backgrounds, including those with mental health challenges, substance abuse issues, or trauma histories.
Excellent communication, organizational, and problem-solving skills.
Proficiency in case management software and standard office applications.
